We welcome any suggestions or comments about this website - our aim is to make it user-friendly.
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill or tenancy agreement along with one form of photographic ID (driving licence, passport)
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. These forms are available at Reception and can be completed when you attend the Practice with your proof of identity and address.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.

A Patient Guide to GP Services
If you live within our catchment area and wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. However, we recommend you to attend the Practice and complete the forms at the Practice as you will need to attend to provide the proof of identity and address before your registration can be completed.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect. Therefore we advise you attend the Practice and complete the pre-registration form and medical questionnaire.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

If you do not reside within the catchment area you may still be able to register as an out-of-area patient but will not be entitled to home visits. Ask At reception for further details.

Purple boundary line shows our catchment area
Orange boundary line shows our outer boundary area. If you currently a registered patient and move out of our catchment area, you can remain registered if you are moving within our outer boundary lines. You may not be entitled to home visits if you live outside our catchment area. You must still inform the Surgery of your new address.
